Documents Required When Applying for Membership in the Istanbul Dental Association

1. Membership Registration Form (to be filled out at the Association)
2. Copy of ID card (front and back)
3. Original diploma or a notarized copy, graduation certificate from e-Government, or Doctor Information Bank registration from e-Government.
4. Academic career certificate, if available (Dr., Dr. Lecturer, Assoc. Prof., Prof.)
5. Copy of specialty certification, if available,
6. Passport-sized photos (2 copies) (Free photo service available at the Association)
7. Certificate of termination of membership if coming from another Chamber region
8. Chamber membership dues and registration fee

“All photocopying services are provided free of charge at the Chamber”

• Turkish citizen dentists who graduated from abroad must have obtained an Equivalency Certificate from the Council of Higher Education (YÖK) and had their diploma registered with the Ministry of Health. A copy of the Equivalency Certificate and the Registration Certificate will be required at the time of registration.

• For foreign nationals to register with the Chamber, they must first obtain a “Work Permit” from the Ministry of Labor and Social Security. A copy of the Work Permit will be required at the time of registration.
